Abstract Background Obesity and vaginal microbiome (VMB) dysbiosis are each risk factors for adverse reproductive oncological health outcomes in women. Here, we investigated the relationship between obesity, bacterial composition, local inflammation bariatric surgery. Methods Vaginal composition assessed by high-throughput sequencing of 16S rRNA genes cytokine levels measured using a multiplexed Magnetic Luminex Screening Assay were compared 67 obese 42 non-obese We further temporal changes microbiota cytokines subset 27 women who underwent Results The component was characterised lower prevalence Lactobacillus -dominant VMB higher high diversity ( spp., Gardnerella - spp. depleted) VMB, with subjects p <0.001). Obese had relative abundance Dialister species <0.001), Anaerococcus vaginalis =0.021), Prevotella timonensis =0.020) decreased crispatus =0.014). Local IL-1?, IL-4, IL-6, IL-8, IFN?, MIP-1? TNF? all among women, however, only IL-1? IL-8 correlated diversity. In undergoing surgery, there no significant overall differences following surgery; 75% these remained at 6 months. Prior to body mass index (BMI) structure; post-surgery significantly BMI than those VMB. Conclusions have different increased Bariatric surgery does not change VMB; greatest weight loss 6-month most likely
